1^st Choice Home Care Services, Inc. is a licensed home care service agency based in Brooklyn, New York. We proudly serve all five Boroughs of New York and Nassau County. When it becomes apparent that your loved one requires more assistance than you can provide, our dedicated team steps in. We maintain constant communication among physicians, case managers, insurance companies, and families to ensure your loved ones receive the proper care they deserve.

NHTD and TBI Waiver Programs

The NHTD and TBI Waiver Programs provide personalized, community-based support for individuals with disabilities or brain injuries, allowing them to live independently. Patients who are eligible for Medicaid may qualify for additional hours and services.

Private Pay Home care

Private home care entails personalized services delivered to individuals within the familiarity of their homes, providing greater flexibility in treatment plans.

Pediatric Home Care

Pediatric care for children with developmental delays and/or physical health challenges.

Enrollment Assistance

We will expedite care and support in your home to keep you out of the hospital or nursing facility. We help individuals who are eligible for Medicare, Medicaid enroll into MLTC, HMO, FIDA, MAP, Long-Term Care Insurance, Health Insurance, Worker’s Compensation Plans, or CHHA.

HHA/PCA
